REFERRALS

Referrals can be made by medical specialists via fax using our referral form or by referral letter. In addition to the download link below, our referral form is also available in the Standard Forms section of the Accuro EMR system.

Notes

All referrals require a recent sent of weight-bearing foot and ankle x-rays.

Once the referral is triaged you will receive a letter confirming its acceptance and outlining treatment recommendations.

You will receive a consultation letter on the date of your patient’s appointment.

All consultations, follow up appointments and surgeries are publicly funded by Alberta Health.
