Patients

Your referral can come from a medical specialist, such as a physician, podiatrist, chiropractor, physical therapist, occupational therapist or nurse practitioner with referring privileges. Prior to your referral, your medical specialist will complete a physical examination and order a set of comprehensive standing x-rays. Once received, your medical specialist will be notified of your referral triage and may receive some treatment recommendations for you to work on while you wait for your appointment. You will be contacted directly by telephone when there is an appointment available for you.

 

How to prepare for your appointment

Please bring with you the following:
— Your Alberta Health Care CARD
— A list of all medications, supplements,
and surgical history
— A translator, if required
— Reading glasses, if required
Download and complete our initial intake form and bring it with you to your appointment. If you prefer to complete the form in our office on the date of your appointment, please arrive 15 minutes prior to your appointment time.


Costs

With valid Alberta Health Care, all appointments are publicly funded through Alberta Health.
